This database contains more than 300 categories of data related to Social Determinants of Health (SDOH).  Categories are organzied across 17 groups (or parent categories), including:

Data are not available for some areas.  State averages reflect the unweighted average of reported values for counties in a state, i.e., NA values are excluded.  Similarly, the United States average reflects the unweighted average of reported values for states.  ZIP Code reflects ZIP Code Tabulation Areas (ZCTA), which are slightly different from ZIP Codes used by the U.S. Postal Service.  For additional information on ZCTAs, see here.  

Reported population in this database are from the American Community Survey (ACS), a nationwide survey conducted every year.  This SDOH database utilizes ACS 5-year estimates.  See here for additional information on the ACS.

It is unclear whether the source will update this database beyond 2020.  We will update when those data are available.
