This database contains model-based categories of various health indicators, including health outcomes (e.g., arthritis diabetes, etc.), prevention (e.g., mammography, doctor visits, etc.), disability (hearing vision disability, etc.), health status (e.g., mental and physical distress, etc.), health risk behaviors (e.g., drinking, smoking, etc.), and health-related social needs (e.g., food security, etc.).
Estimates are based on the Behavioral Risk Factor Surveillance System (BRFSS), Census data, and American Community Survey (ACS) surveys. Data are available for the U.S., states, counties, places (cities), ZCTAs (similar to ZIP Codes), and Census Tracts. Data are not available for all categories in all areas. See here for additional information on methodology. The CDC has discontinued some categories, although the total number of categories has increased over time, from 28 for 2018 (published by CDC in 2020) to 40 for 2022 (published by CDC in 2024).
